The Epidemiological Study of Mental Health Problem in Korea



Abstract
Aiming at inquiring out the possible ways to improve and reinforce psychiatric care, rehabilitation and mental health services, and also at clarifying some questions on the nature of mental disorders in Korean. This intensive study was intended to explore the relationships beween several outcome variables, i.e. length of rehospitalization, frequency of rehospitalization, psychological symptoms, social contact, employment and work performance during the last one year, and some sociodemographic, and past historical variables, medical and psychotherapeutic interventions. This study was carried out applying a structured interview schedule to 137 patients with mental disorders discharged from the psychiatric ward of Seoul rational University Hospital in 1976. The data at the index discharge were obtained from the clinical records of the study subjects. The structured questionnaire which was designed for this study was filled up by one of researchers with information taken from the clinical records. While, follow-up field interview with the relatives of the patients was carried out by two trained-psychiatric nurses from July 1st, 1981 through June 30th, 1982 with a structured interview from of which questions and coding criteria were just matched to the questionaire on index discharge.
Among the 13; study subjects 73 patients(male 38, female 35) were able to be traced. However 59 patients (male 35 female 24) were successfully interviewed. Out of a total 59 cases follow-up interviewed, five male & one female patients were dead during the follow-up period. The average duration of follow-up was 65. 3 months in 53 alive patients (male 30, female 23)
In analyses to explore the possible factors affecting the course of mental disorders, predictor variables were rated on the basis of data obtained from clinical records (data on index discharge), and several outcome variables were selected and rated. And the sex variable was controlled.
The relationship between outcome variables and independent variables are as follows.
1. The length of rehospitalization during the total follow-up period had a statistically significant association with schizophrenia, psychological symptom-thought disorder, previous history of psychiatric admission in male, and schizophrenia, parental loss, previous psychiatric treatment and reason for hospitalization in female.
2. The presence of the history of hospitalization during the last one year had a statistically significant relationship with schizophrenia and previous history of psychiatric admission in ma-le, and schizophrenia, marital status and previous history of psychiatric treatment in female.
3. The presence of psychiatric symptoms during the last one year statistically significantly associated. with schizophrenia in male, and family history of mental illness. in female.
4. The degree of social contact during the last one year had a statistically significant association with schizophrenia overall adjustment before onset and previous history of psychiatric treatment in male, and psychological symptom-antisocial acting out in female.
5. The state of employment during the last one year significantly associated with diagnostic group, schizophrenia and previous history of psychiatric admission in male, and schizophrenia, marital status and previous history of psychiatric admission in female,
6. The degree of work performance among the employed during the last one year did not have statistically significant relationship with any independent variables.
In the auto-correlation between outcome variables, in male the length of rehospitalization during the total follow-up period had a statistically significant relationsphip with other all outcome variables and hospitalization, employment and work performance during the last one year in female.
Among the independent variables, schizophrenia and previous history of psychiatric admission in male, and schizophrenia, previous history of psychiatric treatment and marital status are the variables with a highly significant impact on outcome predication.
